Objective 

The objective of the Emergency Data Card is to assist emergency services personnel to obtain an instant up-to-date, accurate medical and personal information about the accident victim.

This card, to be carried like a credit card, contains the person’s personal particulars such as the person to contact in case of emergency, the blood group as well as current medical condition.

It is extremely useful in emergencies such as motor accidents, heart attacks, fainting spells etc. 

Our aim is to distribute this life saving cards to more than 25,000 Malaysians all over the country and we hope to alert every Malaysian on the importance of carrying the emergency data card with them at all times.

Carry it with you at work, on vacation, or just walking in your neighborhood. You'll feel more secure knowing emergency medical personnel will have access to your data they need to administer appropriate care when there's no time for delay.  

Life Saver Card for family
You may want to protect your spouse, children and other family members by getting the Emergency Data cards for them. Cards are especially important for children because they rarely carry other forms of identification. In case of an accident, the Emergency Data Card helps ensure that the family is quickly notified and that the child receives prompt and appropriate medical treatment.

How to obtain the card - :-

A) Download the card:-

1. It's So Easy! Just download the form, print it, fill in the information required. Be sure to PRINT CLEARLY using COLOR INK only. Put all requested information on the card; separate papers cannot be accepted.

2. Download the data card
